With a change in health status, or advancing age, we may not know how to help ourselves, or our loved ones, adjust to new life circumstances. You are here as you recognize you are at a place where you need to reach out for assistance.
As an Aging Life Care Manager, I will help identify and remove the obstacles that are impeding your path forward. Assistance will be provided with needs assessment, short and long term care planning, information and referral,
guidance for individuals with disabilities, promote aging safely in a least restrictive environment, mitigate risks, as well as provide client and caregiver support. Working together with the identified client(s), family, friends, medical providers, and community partners,
peace of mind and quality of life will be improved for all involved parties.
Please Note: I am available for in-person care in San Diego County and Surrounding Areas,
as well as for remote assistance across the US. If you are out-of-area and desire in-person consultation,
information and referral will be provided free of charge, to assist you or your loved one.
Though at times it may feel your options are limited, life is full of opportunities. It is my job to help you, and your loved ones, figure out which resources to take advantage of, in order to live the life you want. As we age, and the life circumstances of those around us change, it can be difficult to cope with, or watch our loved ones experience, functional limitations or acute/chronic health conditions. A son or daughter who used to be more available is now spread thin, caring for their own children/family, managing a career, perhaps at a distance. I can serve as a valuable local link for your loved one.
We all need someone on whom we can rely at moment’s notice to step in and provide guidance when the landscape of our lives changes. Though things may look different from what you had envisioned, you can still achieve the best version of yourself, at any age or life stage, with someone providing you or your loved ones with the appropriate direction.
We all need short and long term plans to live life intentionally. It often takes a third party to help us look at our life circumstances objectively. Based on a comprehensive biopsychosocial assessment, in collaboration with others you choose to have involved with your welfare, as an Aging Life Care Manager, I will be positioned to assist you with all aspects of your care coordination.
Together, we can anticipate and overcome potential obstacles before they become difficulties, supporting you with where you are in your life’s journey.
